Submit your account application by answering a few questions.

We'll review your application on a first-come, first-served basis. This review process can take time, depending on volume. Once approved, you'll receive a notification at your provided email address. Based on your responses, you may be asked to submit additional information before we can review your application or be offered alternative solutions.

Once your account is approved, you can submit orders immediately. Our supply offerings and quantities may change over time, so please refer back to the store for future updates.

Orders are processed on a first-come, first-served basis. We request that participants limit their orders to one per month. This ensures the program's sustainability and allows us to serve more participants statewide. Thank you for your cooperation.

You will receive a tracking number once you submit your order. If there are issues with your order, communicate with the team via info@spwaz.org. Include your order number; we will get back to you as soon as possible.
